Immunization Policy, Information, and Forms

Students are required to comply with all immunization and vaccination policies by the start of classes. Please see individual policies on measles, mumps and rubella (MMR) and COVID-19 using the links below for more information on requirements. Additionally, our Accommodations and Accessiblity office is available to assist with requests for medical or religious exemptions.
